(CARES) Program

| Mission: |
|
Concerned for the welfare of others, CARES
social workers are dedicated to assisting patients holistically and
providing continuity of care throughout their dental treatment. |
| Program Objectives: |
|
1. To provide education on and linkage to
community resources for new and existing dental patients.
2. To address barriers preventing new and existing patients
from receiving oral health care (i.e., transportation, mental health,
housing, legal, chronic pain). |
| How CARES Helps: |
|
The CARES program offers a continuum of
care for all School of Dental Medicine patients throughout their dental treatment or one of its affiliates (i.e., Women and Children's Hospital of
Buffalo Department of Dental Medicine). Our qualified staff is
available to help you Monday - Friday, 9 a.m. - 5 p.m.

Financial Assistance:
 | We can help you understand your dental insurance,
including Medicaid/Medicare applications. |
 | We can help you schedule a pre-payment or payment
plan for your dental needs. |
 | We can help you to address employment concerns. |
Transportation:
 | We can help link you to community transportation
services so you can get to your dental appointments on time. |
Health Care:
 | We can help you to understand your oral health
concerns; |
 | If you are a caregiver, we can help you with oral
health hygiene instruction and/or provide your caregiver with support
services; |
 | We can get referrals to ensure you have good
nutrition (food stamp program) and connect you with community health
care services; |
 | We can help you find services if you have a physical
or mental disability; and |
 | We can give you information on long-term care
facilities including hospice. |
Mental Health:
 | We can help you communicate with your dentist, dental
hygienist and other dental-related staff; |
 | We can connect you with community mental health care
services; |
 | We can offer you encouragement and support throughout
your dental treatment; |
 | We can provide cognitive behavioral skills training
related to TMD pain or depression |
 | We can help you overcome dental anxiety |
